Platters, tracks, cylinders, and sectors. The combined technology determines how densely bits can be packed on the disk. Have you every noticed an audio CD spins faster when reading toward the inside, and slower near the perimeter? Some hard drives use a different technique where the rpm's are the same but the electronics are faster toward the outside.

 

Data is accessed by blocks, several per sector. Physical block address: cylinder, track (platter), block. Logical block addressing: start at 0 and count up. The hard drive controller has its own firmware, with caching, error detection/correction, block remapping, …

 

Access latency comes from several sources:

seek time
Time to move the read/write head to the correct track and settle.
rotational delay
The platters constantly spin, wait for the block to show up.
